Id like to use mainwp to setup my monitors - but I want to use keyword monitors… is this possibile?
If its not possible to do the setup via mainwp, is it possible to at least sync those sites to mainwp? Want that mostly for reporting purposes)

Yes, it’s possible to create keyword monitors directly in the MainWP AUM extension.
For example, this is how the Add New Monitor looks like when using BetterUptime API:
